You and your common-law partner do not have any children and have lived together for at least three years.

OSAP Affidavit of Common Law Status - With Children and Together for at Least One Year

You and your common-law partner are the parents of a child or children and have been together for at least one year

 

OSAP Affidavit of Indigenous Identity

You identify as Indigenous but official documentation of your Indigenous identity is not available.

OSAP Affidavit of Sole Support Parent Status - Never Married or Widowed

You are have never been married or are widowed, and your child or children will live with you full-time during the academic year.

 

OSAP Affidavit of Sole Support Parent Status - Separated or Divorced and Receives Child Support

You are:

  • separated or divorced from your spouse but do not have a formal separation agreement, divorce judgement or court order,

  • your child or children will live with you at least 50% of the time during the academic year; and

  • you receive child support from your former spouse.

OSAP Affidavit of Sole Support Parent Status - Separated or Divorced and Does Not Receive Child Support

You are:

  • separated or divorced from your spouse but do not have a formal separation agreement, divorce judgement or court order,

  • your child or children will live with you at least 50% of the time during the academic year, and

  • you do not receive any child support from your former spouse.

 

OSAP Affidavit of Marital Status

You and your spouse are married but do not have a copy of your marriage certificate.

OSAP Affidavit of Marital Status - Separated or Divorced With No Dependent Children

You are separated or divorced from your spouse but do not have a formal separation agreement, divorce judgement or court order, and you do not have any dependent children living with you.

 

OSAP Affidavit of No Income

You (the student applying for OSAP) did not earn any income in a specific year.

OSAP Affidavit of Foreign Income

You (the student applying for OSAP) earned foreign income in a specific year but cannot provide the required documentation to prove your income.

OSAP Affidavit of Non-Taxable Income

You (the student applying for income) received non-taxable income in a specific year.

 

OSAP Affidavit of Spousal Income - No Income

Your spouse did not earn any income in a specific year.

OSAP Affidavit of Spousal Income - Foreign Income

Your spouse earned foreign income in a specific year but cannot provide the required documentation to prove their income.

OSAP Affidavit of Spousal Income - Non-Taxable Income

Your spouse received non-taxable income in a specific year.

 

OSAP Affidavit of Parental Income - No Income

Your parent did not earn any income in a specific year.

OSAP Affidavit of Parental Income - Foreign Income
